Welcome to our new blog, we are pleased to announce the launch of Seaside Health Care Services’ new website. Seaside Health Care Services is run by an RN Care Manager – focused on independent living and keeping seniors in their home as long as possible. Senior Care Management and Chronic Care Management are specialties of Seaside Health Care Services, servicing the areas of Laguna Niguel, Dana Point, San Clemente, Mission Viejo, Lake Forest, Laguna Hills, Corona Del Mar, Newport Beach and other South Orange County Cities.
Chronic Care Management is a new program as of January 1, 2015. With Chronic Care Management, Medicare reimburses Primary Care Physicians and outsourced care coordinators such as Seaside Health Care Services.
