Seaside RN Care Management Services is a professional team of Registered Nurses who arranges for and oversees the care of aging adults, individuals with disabilities and persons with injuries of all ages.  Our professional care managers are Registered Nurses experienced in planning and coordinating the care of your loved one.  We assist clients with issues related to their health, illness, functional, psychosocial and legal/financial status.  We are recognized in our  community as professionals who have experience navigating through the multitude of options, bringing clients and families an outstanding combination of quality services leading to improved quality of life and desirable outcomes.

Care Management Services include: 

Completes a thorough in-person assessment of the individual and their needs, leading to a plan of care including a comprehensive plan of action with cost effective recommendations

Acts as an advocate in arranging and coordinating services and treatment, including talking to physicians, physician offices and insurance companies

Home health care services set up and provide oversight into such needs as, personal and grocery shopping, meal preparation, housekeeping, laundry, and transportation

Ongoing monitoring and oversight of the client

Medication management and set up

Attend client doctor appointments and educate client and family on physician reports

Money Management and Bill paying assistance and coordination.

Safety and Security monitoring and set up

Family Liaison and mediation of concerns

Crisis Intervention

Hospital advocacy, facilitate, medical file review and discharge planning

Provide referrals to other related professionals

Community Services Referrals

Concern for abuse, exploitation and neglect

Alzheimer’s and dementia intervention
